Best Testosterone Injection Sites

Although there are multiple places where you can inject testosterone, there are three “basic” and most commonly used sites for injecting testosterone intramuscularly (or whatever other injectable anabolic steroids). I wouldn’t recommend other places, especially for beginners.

  1. Thighs – by far the most common injection site. That’s because it is a very accessible area and you’re having a large muscle that you’re unlikely to miss. It’s usually the first spot to inject testosterone.
  2. Glutes – second popular injection site. It’s a great spot because you are very unlikely to hit a nerve or a vein. Downside – it’s not as accessible when performing self-administration. Use the top outer quarter of your glute area for injection. Top left on the left glute and top right on the right glute.
  3. Deltoids – third most popular injection site. It’s accessible and you can easily reach it, but it’s not as large a muscle. That’s why it can be hard for beginners to find the right injection spot.

Tips to Avoid Post Injection Pains (PIPs)

  • Warm your medication before injecting (to at least room temperature or slightly warmer). An easy and widely used trick that really helps with PIPs.
  • Perform injection with relaxed muscles. Don’t inject into sore muscles. Preferably after a hot shower.
  • Never use more than 2 mL of solution per shot. The less you use – the less likely you are to have pains. Using more than 2 mL is very likely to cause pain.
  • Use lower concentrations. If you’re choosing between 400 mg/mL and 200 mg/mL – you are more likely to experience PIPs with 400 mg/mL due to high concentration.
  • Always rotate your injection sites. The more frequently you use an injection site – the more likely you are to experience pain. Give your muscles time to recover.
  • Massage the injection site both before and after giving a shot. It helps your muscles relax further, so you are less likely to experience pain.
  • Slowly inject – injecting too fast is likely to cause pains. About 30 seconds per mL (cc) is going to be perfect.

Prepare for Performing a Shot

You need to have all your materials ready before attempting to perform the injection. For a single injection, you’re going to need:

  • The medication (obviously)
  • A new syringe (1-2-3 mL, depending on how much you want to administer)
  • Alcohol pads
  • Band-aid (if needed)
  • One large-bore (thick) needle of about 20 gauge for drawing the medication out of the vial
  • One small-bore (thin) needle of about 23 gauge for injecting the medication into the body
  • A container for sharp objects to dispose of the needles

Next, wash your hands thoroughly with enough antibacterial soap and water. Generally, keep in mind that the more sterile and sanitized everything is during the injection performance – the better for you. That’s why try to avoid touching unsanitized items and surfaces after washing your hands until the end of the process. Wipe with the alcohol pads everything that you’re about to use unless it’s sealed.

Also, it goes without saying – never use the same syringe/needle/alcohol pads/band-aid or whatever else twice. Keep everything maximum sterile and sanitized.

Preparing the Syringe

  1. Sterilize the lid of the medication (the cap) with an alcohol pad. Throw away the pad.
  2. Remove the syringe from the sealed package and place it on a flat pre-sanitized surface. Never touch the tip of the syringe even with clean hands.
  3. In case there’s an already large (18-21 gauge) needle you can avoid the next step (4). If there is no needle attached to the syringe or there’s a smaller needle than 21 gauge, remove it by using a twisting motion.
  4. Unseal (but don’t uncap yet) a larger needle (18-21 gauge) and attach it to the syringe. Do not touch with your hands the tip of the syringe or especially the needle.
  5. Uncap the needle already attached to the syringe and pull back the plunger on your syringe to draw air into the syringe. Draw air to the mark that you plan to draw the medication. If you use 1 mL (cc), for example, draw air to the 1 ml (cc) mark.
  6. Insert the needle into the medication bottle with the air inside the syringe. Turn the bottle upside down with the needle inside pointing up, holding up the syringe with one hand and the bottle with the other.
  7. Inject the drawn air into the medication by fully depressing the plunger of the syringe. This increases pressure inside making it much easier to draw out the thick oily testosterone solution. Pull back the needle to make sure that the tip of the needle is remaining below the level of fluid in your vial.
  8. Slowly draw the medication into the syringe by pulling back the plunger. Stop at the correct mark.
  9. Pull the needle out and put down the bottle. Recap the needle.
  10. Remove the large-bore needle attached to the syringe. Unwrap the smaller (thinner) needle (22-23 gauge) and attach it to the syringe. Never touch the tip of the syringe or the needle to keep them clean.
  11. While holding the syringe in front of you pointing up, tap it with your finger to allow all air bubbles inside to go up.
  12. Remove the cap of the needle.
  13. Slowly depress the plunger to force all the air bubbles out. Stop when a tiny bit of solution is seen at the top of your needle. Make sure you don’t inject air bubbles as it could lead to embolism. Recap the needle.

Performing a Testosterone Injection Intramuscularly

So, you have everything prepared and you’re ready to inject. Once again, please do it carefully, keep everything as sterile, and clean as possible.

  1. Sit in a comfortable position. Choose your injection site. As earlier mentioned – the thighs are most popular. Go for the middle third of the thigh on the lateral half. Do not attempt to inject on the inner thigh. There are important blood vessels and nerves. You are very likely to hit one. Wipe the immediate area of the skin at the chosen injection site in an outer clockwise movement from the site. Use a never-before-used alcohol pad.
  2. Remove the cap of the needle. By using your non-dominant hand, spread (stretch) the skin in between your finger and your thumb.
  3. Keep the loaded syringe with your dominant hand like a dart right above the injection site at a 90 degree angle.
  4. With a single fast movement, insert the needle all the way into the muscle. It shouldn’t be too slow or too aggressive. Quickly, simple and delicate.
  5. Instead of rushing to inject it ASAP, you need to draw back the plunger first. With your non-dominant hand draw back the plunger. It’s important to do so to see if there’s no blood coming into the syringe. Inject the medication only in case there’s no blood coming into the syringe. If there isn’t, avoid the next step and go straight to step 7.
  6. However, if there is blood in the syringe, it means you hit a blood vessel. You need to stretch the skin again, pull out the needle and use a cotton swab at the injection entry point. Choose another injection spot.
  7. Testosterone is a very thick solution (like most other anabolic steroids). Therefore, you’ll need to apply some pressure on the plunger to inject it into the muscle. Nonetheless, you need to slowly depress the plunger for injecting the solution. Do it at a slow and controlled pace. It’s normal for a medication to go in slowly. Like about 30 seconds for one mL (cc).
  8. As soon as you fully depress the plunger (the syringe is empty), pull back the syringe with the needle from your muscle. Recap the needle.
  9. Rub the injection site in a similar manner with an alcohol pad. If there’s blood coming out – apply a band-aid or/and a cotton swab.
  10. Make sure you discard the needles in a proper container.
